The deal

Each purchase is one styled QR code for $2 USD, paid once. It never expires, there is no subscription and there are no scan limits. Changing where it points and restyling it are free.

Your links

You're responsible for where your codes point and for following the law where you use them. Don't use Form QR Code for phishing, malware, scams, spam, impersonation or anything illegal. We may switch off codes that do, and send their scans to a notice page instead.

Google Forms

Form QR Code works with Google Forms and any other link but is not made by, endorsed by or affiliated with Google.

Refunds

If your code doesn't work for you, contact us through the support form within 14 days of buying and we'll refund you.

Availability

We work hard to keep every short link online, but Form QR Code is provided as is, without warranties. Our total liability for any claim is limited to what you paid us for the code involved.
